At the time of scheduling your appointment, we will ask for your Patient Information as well as your Insurance Information (Medical & Vision). Please be prepared with your insurance cards when you call our office for an appointment. We will call to obtain your eligibility for your insurance prior to your visit. HOWEVER, IT IS YOUR RESPONSIBILITY TO VERIFY YOUR COVERAGE PRIOR TO COMING IN FOR YOUR APPOINTMENT.

When you arrive to our office, we will have to obtain your signature on some paperwork. In addition, we will be scanning a copy of your Driver’s License (as it is necessary for identification) and your Insurance Cards (Medical & Vision).

Our patients LOVE this new method as it cuts down on a lot of the paperwork and will definitely eliminate some of your "wait time" in our office.
